How Psychotherapy Works: From the First Session to Results

Many people delay seeing a psychologist because they don't know what actually happens in sessions. CBT is a structured approach, and that structure can be described in advance.

The first session: assessing the request

The first CBT session is not devoted to "talking about life in general" but to a specific inquiry: what exactly is troubling the person, for how long, under what circumstances symptoms worsen or ease, and what has already been tried. The therapist forms what is called a case conceptualisation — a working hypothesis about which thoughts, behaviours, and circumstances are maintaining the problem.

By the end of the first session, the client usually receives an honest answer: whether this format of work suits them, what the approximate timeline looks like, and what the preliminary plan is.

The structure of a single session

A CBT session has a predictable structure: a brief check-in on how things have been since the last meeting, a review of homework (if assigned), the main topic of the session, a summary, and a new task for the following week. This structure is not a formality — it is part of the method: it keeps the work moving toward a goal rather than simply "talking".

Homework

Practice between sessions is a key element of CBT. This may include thought records, behavioural experiments, or relaxation exercises. Research shows that clients who complete between-session assignments tend to achieve better therapy outcomes.

How long the work takes

CBT is a short-term approach compared with classical psychoanalysis. The NICE depression guideline mentions courses such as 16 sessions of individual CBT for severe depression, with additional sessions possible for relapse prevention. The exact duration always depends on the presenting issue, its severity, and the goals — it is not a fixed number for everyone.

How to know the therapy is working

In CBT, progress is measurable: the therapist and client define in advance what specifically should change — the frequency of panic episodes, sleep quality, the ability to perform a previously avoided action. This is not a vague sense that "things feel better", but concrete markers that can be tracked.
